As Matt states it’s a limited beta trial. We are an agency in the Uk who have been given access to this for a small number of clients (along with the AdWords Plusbox). We are currently trialling both but yet to see ads show with sitelinks running, although we have the PlusBox working great.

You can have up to 10 sitelinks in your account. They point to specific urls you already have.

Up to a maximum of four will display, but Google only serves them up for a highly relevant, strong Quality Score and CTR keyword. They stress the “brand” related nature of this at the moment.

Yes, it’s a beta function only available to a limited # of advertisers & think it runs on a CPA basis.
